I Have Emulex LPem12000-S 8Gb/s Fibre Channel10df:fc40 10df:fc42 (Firmware Version 202A3)
esxi 6.0.0 (old CPU) (other esxi work norm, on Emulex LPem12000 8Gb/s Fibre Channel all time)
After some period of time, IBM Fibre Channel Disk begin unattach on this esxi.
